COVID-19 VACCINATION CARD: INFORMATION FOR PATIENTS

We have had a number of enquiries regarding the importance of the COVID-19 vaccination appointment reminder card. We would like to reassure all our patients that these cards are not proof of vaccination – they do not contain secure, personal information. If you have been vaccinated all the details are contained on your medical records.

Patients have contacted us regarding misplaced/lost cards. Please be assured these cards are not proof of your vaccination, and they are not Covid certificates or Covid passports: quite simply anyone could amend the written details if they wished.

These cards were created by Public Health England in December as a reminder that patients need two doses of the COVID-19 vaccination. The cards feature a space to list the date and batch number of the first and second vaccination and are only for the patients’ own information/interest.

Any patient still wishing to receive a card when attending their vaccine appointment will be given one, however, for those who have lost or did not receive a card, it is not important as all details are included on the patient records.

You may wish to download the NHS App which enables you to access your records – including full details of your immunisations such as the Covid-19 vaccination – as well as offering health advice and allowing you to order repeat prescriptions. Alternatively, if you have registered for Patient Access, you will also be able to view your medical records and print out your immunisation details.
