Altrincham Healthcare Alliance is a primary care network (PCN) of five local GP practices committed to the greater provision of patient-centred healthcare by working together with community, mental health, social care, pharmacies, hospitals and voluntary services.
Determined to put our 48,000 patients are the heart of all we do, Altrincham Healthcare Alliance PCN includes Altrincham Medical Practice, Park Medical Practice, Shay Lane Medical Centre (Drs Cranston, Haslam and Connell), St Johns Medical Centre and West Timperley Medical Centre.
Altrincham Healthcare Alliance is one of five PCNs in Trafford and is dedicated to providing first-class health and social care to the patients of our five practices. Primary care networks have been developed throughout the country to better meet the needs of patients through enabling personalised, co-ordinated and integrated health and social care. This supports the needs of an increasing population in which people are living longer. Many people are also living with long-term conditions such as diabetes and heart disease, or suffering with mental health issues and may need to access their local health services more often.
The teamwork between our five GP practices, and our connections with local services, is already making a tangible difference to the health and wellbeing of our patients: working together meant that we were the first PCN in Trafford to offer COVID-19 vaccinations to our patients.
